Eligibility and Enrollment Age

This program is available for 18 to 60 yrs. old and active Pag-IBIG Fund Member.

Highlighted Terms

Persons belonging to the following professions shall be entitled to only 50% of the Insurance Benefits, except for Fire and Lightning (cash assistance) benefit:

Professional Racing, professional divers and/or other professional sports
Ship’s crew and/or stevedores;
Aviation crew;
Military service, armed forces and police personnel;
Elective positions (e.g. Mayor, Brgy. Captain, Congressman, etc.)

Coverage Period and Annual Premium

The cover period is 1 Year with an annual premium of Php 120 only.

I'd like to ask if the payment in Lockton is one time only or can I pay in installment basis in a year?
Also, I'd like to ask when is the maturity date or the time that I can avail benefits or claim if ever something happens in the future or what.

Coverage Period - 1 Year
Annual Premium - Php 120 (maximum of 5 COCs)

For Claims Management:

1. NOTIFICATION (Notify claim via e-mail, text, or Viber message)
2. SUBMISSION (Submit complete claim documents)
3. APPROVAL (Receive outcome of claims assessment)
4. SETTLEMENT OF CLAIM (Get claims settlement)

a. Death, Disablement, and Dismemberment:

Certificate of Cover (COC)
Police report and/or Accident report
Death Certificate – LGU issued or Hospital Issued
Insured & Claimant’s Valid ID (with photo and signature)
Hospitalization Records (if confined prior to death)
Additional supporting documents (required by Paramount Insurance)

Ang claims processing ay magsisimula upon submission ng kumpletong documents galing sa miyembro.

b. Hospital Cash Assistance:

Confirmation of Cover (COC)
Medical Certificate
Certified true copy of hospital’s statement of account

- Hospital Cash Assistance – one incident per unit, kung ikaw ay kumuha ng 5 units, ibig sabihin ay may 5 beses ka pwede mag claim for different hospitalization/confinement incidents

- Ambulance Cash Assistance – one incident per unit, reimbursement of paid ambulance transportation

Exclusions: This group policy does not cover loss, or expense of whatever nature, directly or indirectly caused by or resulting from any one of the following:

1. The Insured willfully committing or taking part in crime;
2. The Insured having taken a prohibited drug unless taken in accordance with proper medical prescription and directions and not for treatment of drug addiction.
3. Nuclear radiation or radioactive contamination;
4. Pre-existing conditions;
5. Maternity and Childbirth, abortion, miscarriage or any complications;
6. Sabotage or Terrorism;
7.Any treatment or surgical procedure for Congenital Anomalies, STD, HIV-related diseases or AIDS;
8. Any treatment of mental and nervous disorders, including but not limited to insanity;
9. Long-term rehabilitation and psychiatric care;
10. Any cosmetic or plastic surgery, any dental work, treatment or surgery except when necessary for the alleviation or treament of damage to the insured individual caused solely and directly by accident;
11. Any treatment for obesity, weight reduction or weight improvement and other related treatments;
12. Any experimental procedure or procedures that are not accepted as standard medical procedures including but not limited to chiropractic, acupuncture, herbal medicine, alternative medicine and other similar procedure or treatment;
13. Engaging in political activities and persons holding any elective government position;
14. Hazardous activities such as but not limited to excluded activities mentioned above;
15. Any other exclusion as per printed Personal Accident Insurance Policy.

Ano po mangyayari sa kaagapay program ko po kapag hindi ko ito nagamit sa loob ng isang taon (dahil 1 unit lang kinuha ko)? At paano naman ako makakaextend ng unit kung nakakuha na ako ng 1? Salamat po!

Hello Ana, maraming salamat sa iyong pagbisita at paglalaan ng oras. Kung hindi niyo po nagamit o na-claim ang inyong coverage sa loob ng isang taon ay nangangahulugan lamang po ito na kinakailangan niyo pong i-renew kung nais niyo pa pong ipagpatuloy ang inyong insurance.

Hello Camille, maraming salamat sa iyong pagbisita. Ang Lockton Kaagapay Program ay isang bundled insurance na nagbibigay proteksyon sa di inaasahang pangyayari. Ang coverage nito ay life, accident, hospitalization, at property protection.

Sa halagang 120 peos kada taon, ang bawat miyembro ng Pag-IBIG Loyalty Card o Pag-IBIG Loyalty Card Plus ay maaring makakuha ng microinsurance.

Term Insurance po siya na kung saan ay kinakailangan niyo pong mag-renew kada taon kung nais niyo pa pong ma-insured.

Pag 5 units ang inavail, ibig sabihin 5x ang benefits ang makukuha but for 1 year lang lahat. Then irerenew na po ulit. Tama po ba? and not for 5years. Kase may nabasa ako na previous question was 5units means 5years daw po ba? And you said yes. Kaya po mejo nalito ako. Pakiclear po ☺️
